To support a healthy community network, the FLPPS IT Team, advised by various community partners, is pursuing four main tracts. The tracts include:
Digitization of the Clinical Delivery System
Digitization of the clinical delivery system will lead to interoperability and integration across provider types. In order to achieve this objective, the DSRIP program requires that by 2018 all eligible providers in the FLPPS network must achieve:
- EHR and/or Care Management Implementation (based on provider type)
- Stage II Meaningful Use (limited to Safety Net PCPs)
- PCMH Level 3 (2014) and/or Advanced Primary Care recognition (limited to Safety Net PCPs)
- Interoperability with Rochester RHIO and FLPPS
As clinical providers digitize their systems, a supplemental IT integration system will be established for smaller CBO partners to provide important information about the social determinants of health which support clinical health.
Connectivity with the Rochester RHIO
Once each Partner is digitized, we must then integrate each system with the Rochester RHIO which will be the hub for bidirectional information exchange.
Care Coordination
data integration will help to support collaboration with regional Health Homes, Accountable Care Organizations, and Independent Physician Associations to support care coordination and patient engagement.
Population Health Analytics
In order to effectively impact health outcomes aggregation and stratification of both clinical & non-clinical (social determinants of health) information is imperative. Once this information is gathered into a consolidated data warehouse, FLLPS will be able to identify gaps in care across the network by implementing a population health and analytics platform
The strategic planning and implementation of each IT initiative is guided by the FLPPS IT Committee and operationalized by the FLPPS Central IT team.
