Population Health

As the health care delivery system undergoes transformation towards value based care there is growing emphasis on prevention and population health management, which demands greater attention toward the social determinants driving poor physical and behavioral health patterns.

Population Health is defined as the health outcomes of a group of individuals, including the distribution of such outcomes within the group. 

Population Health Management refers to the variety of individual, organizational and societal interventions to integrate healthcare, public health and socio-environmental determinants of health to improve health outcomes of a defined group of individuals

Population Health is addressed across a number of DSRIP projects, including:

FLPPS is currently developing a Population Health Roadmap that seeks to integrate the work of the DSRIP projects with other population health activities taking place across the service area.
