Increasing Access to Healthcare for Vulnerable People in Steuben & Livingston Counties

Rochester, NY: Finger Lakes Performing Provider System (FLPPS) is collaborating with Catholic Charities of Steuben / Livingston to improve healthcare access in those counties.

Catholic Charities of Steuben / Livingston will expand its outreach and use a mobile van to provide telehealth services and social determinants of health screenings.

Social determinants of health are health-related social needs that have an effect on health outcomes. Examples of social determinants of health include housing, food, transportation, utilities, and personal safety.

The agency will use these screenings to refer individuals to area providers for primary and behavioral health services. Catholic Charities will coordinate and support these individuals in getting needed healthcare and social services.

“FLPPS is proud to partner with Catholic Charities of Steuben / Livingston. They have fostered deep trust with individuals and have the expertise to deliver compassionate services in a collaborative manner. This innovative program aims to address health disparities and will provide vital care to vulnerable individuals in a rural community,” said Carol Tegas, Executive Director of FLPPS.

“During COVID-19 we learned something about the area we serve. We saw a massive increase for assist requests when a delivery model was an option. This opened our eyes as an agency to just how wide-spread poverty was in rural communities due to social barriers. If we are able to eliminate or address some of these barriers it will allow us to serve more people in a more holistic manner,” said Steve Olix, Business Relations Specialist at Catholic Charities Steuben / Livingston.
